*We do not accept Medicare, but do accept most Medicare Advantage plans

** We accept Oregon Health Plan (OHP) Open Card as well as the following OHP plans through Health Share of Oregon for acupuncture only:

• OHSU Health / Moda

• Providence Health Assurance

• PacificSource Community Solutions / Legacy Health

  We do not accept OHP Care Oregon or OHP Kaiser plans.

Quote of Benefits

As a clinic courtesy, we are happy to provide all new patients with a snapshot of your benefits as quoted to us by your insurance company. Upon booking a new patient appointment, you'll receive an email with our intake paperwork where you can include your specific insurance plan information. We’ll send over an insurance quote agreement ahead of your scheduled visit. You can also contact your insurance company directly by calling the member contact phone number located on the back of your insurance card.

Insurance Plan Updates

If you are an existing patient and your insurance has changed since your last visit, you must send your updated information to us at least 2 business days prior to your next appointment.
